Overview: The role of the Pathways Inclusive Education Coordinator and Teacher is multifaceted, supporting students, teachers, Educational Assistants (EAs), and parents. Your primary responsibility is to support the diverse needs of students on your caseload, utilizing the framework of Universal Design for Learning (UDL) to guide your practice. You will provide targeted instruction that aligns with classroom and Individual Education Plan (IEP) goals, working to support, rather than replace, classroom instruction. Additionally, this role encompasses the complex planning, oversight, and collaboration required for the Pathways Program. This involves supporting students whose programming requires a higher degree of individualization, safety consideration, family communication, interdepartmental coordination, and ongoing IEP case management. This role requires classroom teaching; through the Pathways program, students will access English Language Arts, Mathematics, Social Studies, and Science, as well as any other courses as necessary through replacement curricular goals aligned to the student’s present levels of performance and learning needs.

Key Responsibilities & Core Competencies

Direct Student Support & Instruction

  • Provide targeted instruction for Pathways students (students with IEPs who are accessing learning through replacement curricular goals aligned to their levels of performance and learning needs).

  • Provide interventions and direct instruction to support specific IEP goals.  

  • Create social groups and other social-emotional or self-regulation supports for unstructured times.  

  • Conduct diagnostic assessments (e.g., WJIV) if qualified.

IEP & Case Management

  • Serve as the primary case manager for designated Pathways students and your general caseload, overseeing the development, revision, and ongoing monitoring of IEPs.

  • Collaborate, prepare, write, and monitor IEPs and IEP Progress reports.

  • Coordinate the collection and organization of student data, support records, and progress information required for effective IEP development.

  • Create simple, goal-focused Learning Profiles for students requiring extra support.  

  • Monitor timelines and follow-up requirements related to reporting periods, documentation updates, and parent communication.  

Specialized Program & Safety Planning

  • Support the design and review of student programming that reflects identified learning, behavioural, social-emotional, communication, sensory, and safety needs.  

  • Provide frameworks and lead the process of collaboratively creating individualized safety, supervision, regulation, and behavior plans.  

  • Collaborate with staff to identify foreseeable risks and maintain consistent protocols for students with enhanced needs related to elopement risk, medical concerns, or emotional regulation.  

  • Support proactive planning for transitions, risk reduction, and student success across classroom and non-classroom settings.  

Collaboration, Coaching & Mentorship

  • Provide instructional coaching, co-teaching, and research on effective adaptations for all students with IEPs on your caseload.  

  • Suggest interventions to classroom teachers and support them in educational planning.  

  • Provide guidance, resources, and mentorship to staff supporting Pathways students, including Learning Support staff, classroom teachers, and EAs.  

  • Model inclusive techniques and common language, encouraging a trauma-informed, relationship-based, and student-centered approach.  

  • Provide observational and performance data for administrative evaluations of EAs.  

Communication, Liaison & Reporting

  • Serve as a key point of contact between the support team, families, and school administration regarding student programming and case management matters.  

  • Facilitate and lead IEP meetings, and co-facilitate School-Based Team (SBT) meetings with administration.  

  • Support clear, timely, and professional communication with families regarding IEP processes and program implementation.  

  • Maintain organized and confidential records in accordance with school expectations, Ministry compliance requirements, and professional standards.  

  • Conduct self-assessments of job performance, attend conferences, and continuously develop professional expertise in effective instruction and educational leadership.
